Metamorphosis Australia (“we”, “us”) provides NDIS disability support services. We respect your privacy and handle personal information in line with the Privacy Act 1988 (Cth) and the Australian Privacy Principles (APPs). This policy explains what we collect through this website and why.
What we collect
When you submit our enquiry/referral form we collect only what you provide: your name, role (e.g. participant, family member, support coordinator), email, phone, and your message. The form asks you not to include health or medical details.
Why we collect it
Solely to respond to your enquiry and, with your consent, to contact you about the supports you’re asking about. We do not use your details for unrelated marketing, and we do not buy or sell contact lists.
Consent
We only contact you because you asked us to via the form (or because you have an existing relationship with us). You can withdraw consent or unsubscribe at any time by replying to any message or contacting us below.
How it’s stored & who can see it
Submissions are stored securely on infrastructure we control and are accessible only to authorised Metamorphosis Australia staff. Enquiry details are kept separate from any clinical records. We never sell your information and only disclose it where required by law or with your consent.
Access, correction & deletion
You can ask us to access, correct, or delete the personal information we hold about you. We’ll action reasonable requests promptly.
Complaints
If you have a privacy concern, contact us first. If unresolved, you may contact the Office of the Australian Information Commissioner (OAIC) at oaic.gov.au.
